By completing the following form, you will be entered into our database of volunteers.  This form is in no way a binding contract for your involvement with MUM.  We understand that your schedule is always changing.  The purpose of this form is only to give us a way to organize and coordinate our efforts more efficiently.  We do request however that only those serious about serving in this capacity complete the MUM volunteer form.  We will do our best to communicate with you about opportunities and we thank you for your commitment to service.
